Information we would collect
In a live deployment, an appointment request would collect your name, phone number, email, chosen treatment and preferred visit time so the clinic can confirm your booking.
How it would be used
Solely to arrange, confirm and follow up on dental appointments and to answer enquiries. It would not be sold or used for unrelated marketing.
